What is an Obstetric ICU?

Why is obstetrical care important?

A pregnant woman’s doctor considers all aspects of labor, delivery, and postpartum care suitable for the patient.

At OCCU the doctor knows approximately when the labor begins by understanding the interpreting signs that a woman is approaching labor. One of these signs is when a woman’s “water breaks,” where the fluid-filled sac surrounding the fetus ruptures. Water breaking is not always a sign that woman will go into labor soon, labor can still occur weeks later post breaking of water.

Providing pain relief

  • Analgesics and anaesthetics. Analgesics offer pain relief without total loss of feeling. An anaesthetic blocks all feeling, including pain. Epidural or spinal anaesthesia is often administered by an anesthesiologist.

To decide if a cesarean delivery is needed

  • Cesarean delivery or c-section is the delivery of a fetus through a surgical incision made through the abdominal wall and the uterine wallPostpartum care.

At OCCU Labor may be induced (or started artificially) for a variety of reasons:

  • Due to high-risk factors, such as hypertension, preeclampsia, heart disease, gestational diabetes, or profusely bleeding.
  • If the fetus does not receive enough nutrients and oxygen
  • If the woman’s water breaks, but labor does not start within 24 to 48 hours or if the pregnancy goes beyond 42 weeks

Post delivery care

  • The patient is kept under obstetrical care even post-pregnancy as obstetrical services in the hospital would manage any complications that occur, such as infection, haemorrhage, high blood pressure, blood clot formation, the opening of incisions, or breast problems until 6 weeks after delivery.
  • The woman could be referred for help with postpartum depression or other problems.

Who requires OCCU?

A pregnant woman is more prone to develop life-threatening complications initially or at the final stage of pregnancy without a warning.

The process of Shifting of a Patient from Ward to Obstetric ICU is as follows:

  • The consent of the family/companion of a pregnant woman is taken firstly, to begin the procedure.
  • Examination of the current condition and previous medical reports are done thoroughly.
  • Obstetric staff is prepared to depend on the patient’s condition
  • The patient is shifted as it is to the OCCU without disturbing the ongoing treatment.
  • Patient’s medical condition is continuously monitored and follow up is made from time to time.
  • Adequate airway is provided to the patient
  • Baby is shifted along with the mother to the OCCU post delivery if found safe enough for both.

The OCCU set-up is managed by the anesthesiologists, but the decision of admitting the patients to OCCU is governed by the obstetric team. All critically ill obstetric patients who require hemodynamic monitoring and vasopressor support, invasive or non-invasive ventilator care and also patients with severe organ dysfunction are admitted to the OCCU. Medical and surgical consultations are taken when required
